Treatment options for subjective tinnitus

Treatment options depend on whether the tinnitus is acute or chronic.

Acute-stage tinnitus is usually treated for a few days with Prednisolon, and also with infusion therapy. The aim is to thin the blood, and hence to improve the blood flow in the inner ear. So far, no statistically verifiable effects could be demonstrated for tinnitus. Other, less frequently used medications for acute tinnitus are Procain and Lidocain. Hyperbaric oxygen therapy can also be helpful, but is now recommended as a backup therapy only.

There is no conclusive proof for the efficacy of anti-depressants in the treatment of acute-stage tinnitus. Only Amitriptylin has been shown to have a long-term effect in clinical trials. Tranquilizers such as Diazepam only helped temporarily - and should not be taken for longer periods anyway, due to the known dangers of addiction. Tocainid can also be tried for acute and sub-acute tinnitus.

Non-medicinal treatment of chronic tinnitus

At present, few studies of apparatus-based acoustic therapies permit scientifically correct statements with regard to their effectiveness. Only two studies recorded a partially significant improvement when using a hearing aid or tinnitus masker. Biofeedback can be effective in individual cases.

Behavioral health psychotherapy has shown positive therapy results. Since most studies were conducted without controls, however, it is difficult to scientifically evaluate the outcomes published to date. One study involving a combination of tinnitus retraining therapy and group behavioral therapy demonstrated a stable improvement that lasted for over six months.

Overall, treatment options for tinnitus are limited. No treatment has been proven to be generally effective.
